There are 3 ways to register for the AADSM 21st Annual Meeting:

Member: To register as a member, the registrant must currently be an individual member of the American Academy of Dental Sleep Medicine (AADSM). If the AADSM is unable to confirm that the individual is a member of the society, the AADSM will register the individual as a nonmember for all sessions at the prevailing fees.
Nonmember:If you are not an individual member of the AADSM and would like to become a member, please complete the membership application form in this program. For more information regarding AADSM membership, call the AADSM Membership Department at (630) 737-9705.
Dental Staff of Nonmember:If your employer is not an individual member of the AADSM and would like to become a member, please request that he/she completes the membership application form. For more information regarding AADSM membership, call the AADSM Membership Department at (630) 737-9705.
Guest Passes:A registered attendee may elect to buy a guest pass. These guest passes are for family members only and allow entrance to the exhibit hall and industry sponsored events only. You must be 16 years of age in order to enter the exhibit hall. List the first and last name of each registered guest on attendee’s registration form.

If you wish to become a member and take advantage of the membership benefits, you must submit your membership application at the time of submission of your AADSM registration form. Notification of cancellation must be submitted in writing to the AADSM national office. A $50.00 administrative fee will be withheld on the cancellation postmarked on or before May 23, 2012. Refunds are not possible after this date.
The AADSM, its contractors and attending news media may be photographing or videotaping, scientific sessions and events at the Annual Meeting. By attending the 21st Annual Meeting, attendees acknowledge these activities and agree to allow their image to be used by the AADSM in association publications, on the meeting website and in marketing and promotional materials. Attendees at the 21st Annual Meeting waive all claims against the AADSM for any liability resulting from these uses.
By submitting your registration form for the AADSM 21st Annual Meeting, you agree to the membership disclaimer, cancellation policy and participation disclaimer terms and conditions stated above.
In order to attend educational courses on the morning of Thursday, June 7 or meet the professor sessions on Thursday, June 7 through Saturday, June 9, you must be registered for the general session. The AADSM does not offer registration to only attend educational courses or meet the professor sessions. These sessions have limited seating. Topics and speakers for the sessions are available in this program. The price of the meet the professor sessions covers the cost of your meal only.

Tickets are sold on a first-come, first-served basis; advanced purchase of tickets is strongly encouraged. The AADSM does not have a waiting list for sold-out educational courses. If the educational course you select is full when your registration is received, you will not be charged the additional course fees.
In order to attend meet the professor sessions on Thursday, June 7 through Saturday, June 9, you must be registered for the general session. The AADSM does not offer registration to only attend meet the professor sessions. Space is limited and meet the professor sessions sell-out quickly. Tickets are sold on a first-come, first-served basis; advanced purchase of tickets is strongly encouraged. The AADSM does not have a waiting list for sold-out meet the professor sessions. The price of the meet the professor sessions covers the cost of your meal only. If the meet the professor session is full, you will not be charged or will receive a refund.
After registering for the AADSM 21st Annual Meeting, registrants will receive confirmation from the AADSM via email. It is recommended that you bring your confirmation notice with you to the meeting. This will allow you to pick up your registration materials by using ExpressPass®. If there are any questions or discrepancies with your registration, the confirmation notice will provide the information necessary to quickly resolve the complication.
